Are the local agencies and service providers required to use form JFS 03002 WIOA Youth Program Eligibility Application?

Answer:

Yes. It is required for all CCMEP participants and must be completed prior to the assessment and IOP. OAC rule 5101:14-1-02. (Updated: 12/13/2017)
