Please complete one form for each assistance program where you suspect fraud may be occurring. It is important to include as much information as possible; however, you may remain anonymous. If you provide your name and contact information, state or county fraud investigators may contact you for additional information.
Privacy laws prevent us from discussing the results of any investigation that may result from your report.

If you suspect impropriety or wrong doing in regards to Accounting, Fraud, or auditing practices, please report that to the Ohio Auditor of State (AoS) or Office of Budget Management (OBM) using the buttons provided here, or by calling: 1-866-FRAUD-OH
If you suspect that your personal information has been stolen and/or if you received a 1099-G tax form from the Ohio Department of Job and Family Services (ODJFS) and did not apply for unemployment benefits in 2020, the agency needs to hear from you immediately.
