Question:
Must CCMEP forms for minors be signed by both the participant and parent/guardian?
Answer:
CCMEP forms should be signed by both whenever possible. However, if a parent or guardian is not available to sign, document reasons why a parent/guardian was unavailable in OWCMS case notes.
