| The Contribution Section processes forms and correspondence via an electronic imaging system. Your documents are scanned, converted to an optical image, and then processed by an automated workflow system. Please type or print clearly and legibly with black ink on all forms and correspondence. | If you have had no employees and paid no wages for any calendar quarter, you may file your quarterly Contribution and Wage Report by telephone. The toll free number is 1-866-44-UC-TAX (448-2829). You will need to have both your ODJFS employer number and your federal employer identification number (FEIN) in order to file your report. |

- JFS-66300 (Report to Determine Liability): Used by employers to apply for an unemployment compensation tax account. Also includes form JFS-66300A (Transfer of Business), to be submitted if an employer has acquired any portion of a business.
- JFS-00503 (Application for Voluntary Successorship: Transfer of Substantially All Assets): If an employer acquires substantially all (75%) of a trade or business and immediately after the transfer employs substantially the same (75%) individuals who were employed in the acquired trade or business, the employer may become a successor-in-interest upon filing an application signed by both the transferor and transferee within 90 DAYS of notice to the transferee that the request may qualify for a transfer of experience. This is voluntary under the law and under this type of transfer, the successor-in-interest assumes all of the resources and liabilities of the transferor's account including the transferor's entire employment experience.
- JFS-00502 (Application for Voluntary Successorship: Transfer of Clearly Segregable and Identifiable Portion): If an employer acquires a clearly segregable and identifiable portion of a trade or business and employs substantially the same individuals who immediately prior to the acquisition were employed in the segregable and identifiable portion, the employer may be assigned a proportionate share of the predecessor's account by filing an application, signed by both the transferor and transferee within 90 DAYS of notice to the transferee that the request may qualify for a transfer of experience. This is voluntary under the law and under this type of transfer, the successor-in-interest assumes only the resources and liabilities of the transferor's account, including the employment experience, that is attributable to the segregable and identifiable portion transferred.
- JFS-66308 (Disposition of Business): Used by employers to report a change of ownership or to close their unemployment compensation account.
- JFS-66105 (Employer's Contribution Report and Report to Determine Liability): Used by employers to amend information previously submitted on the bottom section of the Employer's Contribution and Wage Report (JFS-66111 or JFS-66112) and by new employers in reporting initial wage information. This form should only be used as a last resort to substitute for the bottom section of the Employer's Contribution and Wage Report (JFS-66111 or JFS-66112) when that form is lost or damaged. Note: the JFS-66105 cannot be used to substitute for the top section of the Employer's Contribution and Wage Report (JFS-66111 or JFS-66112).
- JFS-66113 (Employer's Report of Wages - Supplemental): Used by employers to report original wage information for more than twenty employees and by new employers in reporting initial wage information. Unfortunately, due to the use of high-speed optical character recognition scanners we are not able to offer a downloadable version of this form. However, if you have an active employer account with ODJFS, we do offer the ability for employers to file electronically at https://ohiobusinessgateway.ohio.gov/hb202/login/login.asp
- JFS-66209 (Employer's Claim for Refund): Used by employers to claim a refund or adjustment of an overpayment. Everyone who submits a claim for refund must also attach a JFS-66210 (Wage Information in Support of a Claim for Refund).
- JFS-00501 (Employer's Representative Authorization): Used by employers to authorize someone other than the employer to provide information pertaining to Unemployment Compensation Taxes and Unemployment Benefits. Also, used by employers to authorize the Ohio Department of Job and Family Services to furnish information directly to a representative.

Where can you get extra forms? If you need contribution or wage record reporting forms, any Compliance office will provide them at no cost. For those employers with more than 20 employees, supplemental pages may be obtained from Compliance offices, ordered from the Contribution Section, PO Box 182404, Columbus, Ohio 43218-2404, or by calling the Contribution Section at 614-466-2319. |
