Unemployment Compensation Tax Reporting & Related Forms  
The Contribution Section processes forms and correspondence via an electronic imaging system. Your documents are scanned, converted to an optical image, and then processed by an automated workflow system. Please type or print clearly and legibly with black ink on all forms and correspondence. White paper works best, as well. Outstanding customer service is our mission and our efficiency is enhanced by your adherence to these instructions.
 
If you have had no employees and paid no wages for any calendar quarter, you may file your quarterly Contribution and Wage Report by telephone. The toll free number is 1-866-44-UC-TAX (448-2829). You will need to have both your ODJFS employer number and your federal employer identification number (FEIN) in order to file your report.
 
Listed below are several of our most requested forms. When appropriate, instructions for completing the forms have been included. For new employers registering their account online or existing employer to manage their account online, please visit www.unemployment.ohio.gov.

  • JFS-20100 (Report to Determine Liability): Used by employers to apply for an unemployment compensation tax account. To obtain your account number immediately, please visit www.unemployment.ohio.gov to register your unemployment account.
     
  • JFS-20101 (Transfer of Business): To be submitted if an employer has acquired any portion of a trade or business and/or both employers are under substantially common ownership, management or control. To submit this request online, please visit www.unemployment.ohio.gov.
     
  • JFS-20118 (Application for Voluntary Successorship: Transfer of Substantially All Assets): If an employer acquires substantially all (75%) of a trade or business and immediately after the transfer employs substantially the same (75%) individuals who were employed in the acquired trade or business, the employer may become a successor-in-interest upon filing an application signed by both the transferor and transferee within 90 DAYS of notice to the transferee that the request may qualify for a transfer of experience. This is voluntary under the law and under this type of transfer, the successor-in-interest assumes all of the resources and liabilities of the transferor's account including the transferor's entire employment experience. 
     
  • JFS-20119 (Application for Voluntary Successorship: Transfer of Clearly Segregable and Identifiable Portion): If an employer acquires a clearly segregable and identifiable portion of a trade or business and employs substantially the same individuals who immediately prior to the acquisition were employed in the segregable and identifiable portion, the employer may be assigned a proportionate share of the predecessor's account by filing an application, signed by both the transferor and transferee within 90 DAYS of notice to the transferee that the request may qualify for a transfer of experience. This is voluntary under the law and under this type of transfer, the successor-in-interest assumes only the resources and liabilities of the transferor's account, including the employment experience, that is attributable to the segregable and identifiable portion transferred.
     
  • JFS-20110 (Disposition of Business): Used by employers to report a change of ownership or to close their unemployment compensation account. To deactivate your account online, please visit www.unemployment.ohio.gov.
     
  • JFS-20125 (UC Quarterly Tax Return): Used by contributory employers to submit quarterly wage detail and unemployment taxes. To submit your quarterly tax report online, please visit www.unemployment.ohio.gov.
                  
  • JFS-20125 (UC Quarterly Tax Return): Used by reimbursing employers to submit quarterly wage detail information. To submit your quarterly tax report online, please visit www.unemployment.ohio.gov.
  • JFS-20106 (Employer's Representative Authorization) for Taxes:  Used by employers to authorize someone other than the employer to provide information pertaining to Unemployment Compensation Taxes. Also, used by employers to authorize the Ohio Department of Job and Family Services to furnish information directly to a representative. To authorize a representative online, please visit www.unemployment.ohio.gov.

  • JFS-00501 (Employer's Representative Authorization) for Benefits: Used by employers to authorize someone other than the employer to provide information pertaining to Unemployment Benefits. Also, used by employers to authorize the Ohio Department of Job and Family Services to furnish information directly to a representative.

  • JFS-20129 (Request to Amend the Quarterly Tax Return): Used by employers to amend an original quarterly tax return. To amend an original report online, please visit www.unemployment.ohio.gov.

  • JFS-20132 (Waiver Request): Used by employers to request a waiver of forfeiture and interest assessments resulting from filing a late quarterly tax return. To request a waiver online, please visit www.unemployment.ohio.gov.
