Ohio Access Success Project

The Ohio Access Success Project (Success Project), part of the Ohio Department of Job and Family Services' Bureau of Home and Community Services, expands Ohio's capacity to serve long-term care consumers in the community by identifying individuals living in a nursing facility who desire to live in a community-based setting and who can do so with linkages to community services and supports.

Funded through a combination of federal grant funds and state funds, the project provides qualified nursing home residents with assistance in making plans for relocation from the nursing facility to a community-based setting; assistance with linking to needed supports and services, such as housing, transportation, financial assistance programs and supportive services; and one-time funding to assist with relocation expenses.

To be eligible for the program an adult must:
  • Be a resident in a nursing home
  • Be Medicaid eligible
  • Need the care that is provided in a nursing home; and
  • Have care needs that can be provided in the community at no more than 80% of the average Medicaid cost of care in a nursing home.

ODJFS has contracted with Easter Seals, the Center for Independent Living Options, and the Brain Injury Association of Ohio to implement the Success Project. The project began in May, 2004 and was piloted in Delaware, Fayette, Franklin and Pickaway Counties. As of February, 2005 relocation teams are available statewide to work with participating consumers to determine what supports and services they will need to move to a community-based setting.
