Case Management Services

What are Case Management Services?

  • A service which includes case management, overall care coordination, and data collection for the determination of program eligibility and cost cap assignments.
  • Provided to consumers on the Ohio Home Care Waiver, the Transitions MR/DD Waiver and the Transitions Carve-Out Waiver benefit packages.
  • Provided by a single case management agency, CareStar. CareStar is under contract with the Ohio Department of Job and Family Services to serve all eighty-eight (88) counties in Ohio through four (4) regional offices.

What does the case management agency do?
  • Visit applicants in their home to gather information to help ODJFS decide if they are eligible for home care services, and determine if applicants can be served safely in their home and communities.
  • Help applicants who are approved for services plan for their care and arrange for providers who can meet their needs.
  • Coordinate overall service provision with all involved providers and systems, for example, local schools and MRDD boards.
  • Assure consumer health and safety.
  • Maintain regular contact with consumers to make sure their needs are being met and that they are still eligible for services.
  • Troubleshoot problems for consumers.
  • Gather information that helps ODJFS administer home care programs.
