Corrective Action Plan
A Corrective Action Plan is the written response from you, the Medicaid home care provider, to a Notice of Operational Deficiency (NOD). The NOD describes a problem, cites an Ohio Administrative Code rule violation, and requires an explanation of what action you must take to correct and/or prevent the problem from occurring again. Sometimes the NOD will ask you to submit specific information or forms, or to explain a policy or process. Generally, you are given 30 days to respond.
In the Corrective Action Plan, you explain to the ODJFS Bureau of Home and Community Services that you understand the rule and how you will assure the situation reported in the NOD will not occur again. If you disagree with the circumstances described in the NOD, please help us by sending additional information and an explanation.
When the Corrective Action Plan is accepted by our office, we will respond with a letter. If we do not accept your Corrective Action Plan, we will also respond.
