Choices Waiver Program
The Choices Waiver program consists of adult day health programs, alternative meal services, environmental accessibility adaptations, home care attendants, home-delivered meals, personal emergency response systems, pest control, and specialized medical equipment and supplies.
Who is eligible?
Consumers eligible for the Choices Waiver program must be eligible for Medicaid, need at least an intermediate level of care, be age 60 or older, and live in an approved service area (this program is not available statewide). Please check with your local regional Area Agency on Aging to see if you live in an approved service area. Three local Area Agencies on Aging act as regional administrators and provide case management services
To be eligible, consumers must also attend a training and be willing and able to direct provider activities and negotiate rates within the program’s cost caps.
