BCFTA Data Requests

Effective August 29, 2011, all information requests will be sent to BCFTA via the BCFTA Data Request Document. Those requesting information are to complete the document and e-mail the request to BCFTA_Data_Requests@jfs.ohio.gov. This mailbox will be utilized specifically for these requests. Information about completing the Data Request is reflected on the document. If you want to keep a record of your request, you can save the completed Data Request as a .pdf document and e-mail your request to the mailbox referenced above.
