The Ohio Department of Job and Family Services (ODJFS) seeks to make it easier for persons to request public records from the department. If you wish to request a record from ODJFS please click onto the link provided below to complete and submit a request. Please explain in detail what record(s) you are requesting. Be as specific as possible as this will assist us in identifying and locating the record being requested. Please specify whether you wish to have the information mailed to you, faxed to you, or if you wish to review the information in a department office. Also specify if you want the information in a specific electronic format. We also ask that you provide your mailing address or e-mail address and telephone number where you can be reached if further clarification is needed regarding the request. Please note that ODJFS may charge a cost for copying the records. The cost may include $.05 per page for paper copies, the actual cost of computer discs or other equipment needed to copy the information to an electronic medium, and the actual cost of computer time if the information is downloaded from a computer system.
