 Forms Online

A registered apprenticeship sponsor can enroll apprentices and update their status using the RAPIDS automated system. Pending access to the system, the sponsor can file these transactions by using the filliable forms listed below:

  1. The Voucher of Credit for Apprentice's Previous Experience;
  2. The Apprenticeship Agreement;
  3. The Application for Certificate of Completion of Apprenticeship.

The sponsor's authorized representative can obtain a RAPIDS user name and password by emailing a request to apprenticeship@jfs.ohio.gov.
