These links provide access to useful documents:

  1. A template with the basic outline of the program standards, or plan, that must be submitted to register an apprenticeship program in Ohio,
  2. A guide to the Equal Opportunity provisions required in the program standards,
  3. A credit voucher [pdf or Word] to be used for validating credit for an apprentice,
  4. The Apprenticeship Agreement form [pdf or Word],
  5. The Apprenticeship Cancellation form [pdf or Word], and
  6. The Application for an Apprentice Completion Certificate [pdf or Word].

Prospective program sponsors can download the standards template and add the appropriate details for submittal to the nearest apprenticeship service representative (see contact list below).  The representative will review the document and forward it to the Ohio State Apprenticeship Council (OSAC).  OSAC approval of the standards constitutes registration of a program.

Existing program sponsors can use the template as a format for program revisions.  Sponsors can also download the Apprenticeship Agreement form, credit voucher, Apprenticeship Cancellation form, and Certificate Application, and fill them out for submittal.  These forms are available in a PDF and MS Word format, and in each case can be completed in two ways:

  • They can be filled out electronically on any computer with Word and/or Acrobat writing software, and then mailed or e-mailed to the apprenticeship service representative.
  • Sponsors without this software can print the blank forms and fill them out by hand.
