Ohio Home Care Providers:
To be enrolled to provide waiver services as an RN, LPN, Independent Daily Living Aide, or Independent Daily Living Non-Aide, or to enroll to perform services with the Ohio Home Care waiver program such as Home Modifications, Supplemental Transportation, Out of Home Respite, please contact your county's Case Management Agency office.
