The web-based system allows the creation of both ETA Forms 750 and 790 electronically and the option to save and reuse the information for future applications. Additionally, the system provides the capability to track applications as they move through the process.

The original application should be filed with the U.S. Department of Labor, Employment & Training Administration, Division of Foreign Labor Certification, 844 North Rush Street 12th Floor, Chicago, IL 60611. The Ohio Department of Job and Family Services is responsible for housing inspections. To schedule an appointment, the employer may call or e-mail.  Please see our Contact Information for the number or address. Housing inspections should be scheduled at least 30 days before the date of need.

If the housing will accommodate more than four people, a camp license is necessary. The employer must call the Ohio Department of Health at 614-644-8605 to make arrangements for a Health Department inspection in addition to the ODJFS inspection.

The State of Ohio will conduct prevailing wage surveys. Employers are encouraged to respond.
