Ohio Medicaid Provider Termination List and Exclusion List

The Ohio Department of Job and Family Services (ODJFS) maintains a list of providers whose Medicaid provider agreement has either been terminated or the provider has been placed on an exclusion list. ODJFS maintains a list of providers whose Medicaid provider agreement has been temporarily suspended. This page contains links to both lists.

ODJFS terminates a provider agreement when:

A provider's license or authorization to provide services is made inactive, terminated, suspended or revoked by any authorizing board or entity; A provider has failed to comply with program requirements specific to the Ohio Medicaid program; A non-agency waiver provider has been convicted of a criminal offense, not related to the Medicaid program, which makes them ineligible to provide services for a period of time;or ODJFS determines that continued participation of the provider is not in the best interest of Medicaid consumers or the state of Ohio.
ODJFS terminates a Medicaid provider agreement and places a provider on an exclusions list when:
A provider has been convicted of health care fraud; The Medicare program has initiated an exclusion of a provider that is binding upon the Medicaid program; A provider has pleaded guilty to, or was convicted of, a criminal activity materially related to the Medicare or Medicaid programs; or The federal Department of Health and Human Services initiates an action that is binding on a provider's participation in the Medicaid program.
If an individual is on the exclusion list, the individual may not be an owner in whole or in part, officer or partner, authorized agency, associate, manager, or employee of a Medicaid provider, The individual is prohibited from owning, contracting for, arranging for rendering or ordering services for Medicaid recipients or receiving direct or indirect reimbursement of Medicaid funds in the form of salary, shared fees, contracts, kickbacks, or rebates from or through any participating provider or risk contractor. The following list contains Medicaid providers whose Medicaid provider agreements have been terminated and Medicaid providers who have been excluded from the Ohio Medicaid program. Most of the providers on the list are not excluded from the Ohio Medicaid program. Employers should look at the final column to determine why an individual appears on the list. The list does not contain voluntary resignations.
