How to Become an Ohio Home
Care Provider
If you are interested in becoming an
independent home health aide, you must have an Ohio Home Care
Waiver or Transitions Waiver consumer who wants your services. If
so, please call the Case Management
Agency that serves the county in which you want to work for an
application packet. A criminal background check through the Bureau of Criminal
Investigation and Identification is required.
If you are interested in becoming an Ohio Home Care Provider (home
health agency or independent R.N.), please contact the ODJFS
Provider Network Management Section at 800-686-6108 (in state) or
614-726-3288 (out of state) and select the Provider Enrollment
response, or on the Web with this link: Provider
Enrollment.
If you would like to provide out-of-home respite, you must already
be identified in the Medicaid computer system as an ICF-MR or a
