Children (up to age 19) and pregnant women can get free health coverage through Healthy Start.
Healthy Start covers the following services:
- Doctor visits
- Immunizations
- Substance abuse

- Prescriptions
- Vision services
- Mental health
|
- Hospital care
- Dental care
- Other Services

Families can apply for Healthy Start filling out the Combined Programs Application (CPA) - JFS Form #07216. By calling the Consumer Hotline at 1-800-324-8680, hotline staff can provide general information or offer assistance in filling out the application. The hotline is staffed seven days a week. Families can also find applications at their local office of job & family services.

For families between 151% - 200% of the federal poverty level (FPL)
- Available to children up to age 19
- Cannot have health insurance. Health insurance is defined as having health insurance that covers
BOTH inpatient hospitalization and
physician coverage - regardless of premiums,
deductible and co-payments.
- Must reapply every 12 months - income changes may affect eligibility status.

For families up to 150% FPL
- Available to children up to age 19 and pregnant women
- Health insurance from another carrier does NOT affect eligibility
- Pregnant women are eligible during the entire pregnancy up to 60 days after the baby is born.
- Babies born to a mother on Healthy Start are automatically eligible for free health coverage for 1 full year.
- Must reapply for coverage every 12 months - income changes may affect eligibility status.
