Application Process

Applying for Healthy Start & Healthy Families health coverage through the Medicaid program is easy. Children, pregnant women & families can apply for Healthy Start & Healthy Families by filling out JFS Form #07216 - the Combined Programs Application (CPA). No face-to-face interview is required when applying for health coverage using the CPA. You can get the CPA by:

  • Visiting your county department of job & family services office,
  • Calling the Medicaid Consumer Hotline at 1-800-324-8680 (TDD 1-800-2923572), OR
  • Downloading an application from our website at http://jfs.ohio.gov/ohp/Application.stm.

The following items will need to included with the application:

  • Proof of Income,
  • Proof of Other Health Insurance (if applicable),
  • Proof of Pregnancy (if application,
  • Limiting Physical Factor* (if applicable), and
  • Immigration Status (if NOT a U.S. citizen).

REAPPLICATION PROCESS

Once a person(s) is determined eligible for health coverage, a reapplication date will be set. Reapplication cycles differ depending on eligibility category. The following show how often the reapplication cycles occur:

  • Healthy Start for Children - Every 12 months.
  • Healthy Start for Pregnant Women - No reapplication required. Covered up to 60 days after baby is born.
  • Healthy Families (for entire family - parents & children) - Every 6 months.
*Resources and limiting physical factor (i.e., age, disability) must be verified if application is made for health coverage through the Medicaid program.
