    1. This label template can be used to create medication bags for children in your program who require medication to be given while in care.  The label can be completed and affixed to a zip lock bag.  Included in the bag would be the medication, the Administration of Medication form (JFS 01217), a dosaging spoon and a health care plan (if needed). 
    2. It is not required that programs use these labels.  They are being provided as a resource for programs that would find this a helpful system for assuring medication is being given in a safe manner.  The labels are 3 1/3" X 4" in size and are compatible to Avery 5164/8164 labels.
  6. Transportation Vehicle Guidelines
